Joining the NCL CareLink Network means joining the only network of its kind in the country. Our network is made up of more than 6000 Registered Nurses — and it's growing weekly.
The nurses in our network provide functional assessments and care coordination services. During our face-to-face assessments, we:
- Evaluate the client's cognitive status
- Assess ADL and IADL dependencies
- Examine health status and living environments that may impact their ability for self-care and independent living.
